Lyft 

Lyft Pass for Healthcare: You can request a ride to and from your medical appointment via the Lyft app. 

To request a free ride, contact your healthcare provider or Medicaid (if applicable) to determine eligibility and involvement in the Lyft Pass for Healthcare program. 

Lyft rides to COVID vaccine appointments: If you need getting to a COVID vaccine site, request a free or discounted ride

Medicaid 

Non-emergency medical transportation: Medicaid may provide reimbursement for non-emergency medical transportation. 

Personal Mileage Reimbursement: When public transportation is not available or does not meet your needs, you may be reimbursed personal mileage at 18 cents per mile up to $300 a month.

ModivCare Services

If you do not live in an area served by bus, paratransit services or you need door-to-door service, you may be eligible for ModivCare services. 

Contact ModivCare to schedule your ride at least 3 business days before your medical appointment.

Provides information and resources regarding bus passes, gasoline vouchers, and transportation expense assistance.

UTA Reduced Farepay Card

Provides a 50% discount off the public fare for all who qualify. 

Bring proof of identification, a current photo to be printed on the reduced FAREPAY card, proof of income, and a medical provider’s disclosure signed by your healthcare provider (for those with a disability).

    • Mileage Reimbursements: A voucher program for persons with disabilities. Vouchers allow Utahns to arrange rides to medical appointments with a driver of their choice who are then reimbursed $.35 per mile by UTA

Salt Lake County Health Department Car Seat Program
    • Sells new car seats on a sliding fee scale to qualifying families in a range of $15-150.
    • To purchase a car seat, you must attend a car seat class and provide proof of qualifying income.
    • To qualify, family income must be at or below 195% of the current federal poverty level.
    • Also includes safety tips, videos, classes and safety checks.
