  • Non-emergency medical transportation: Medicaid may provide reimbursement for non-emergency medical transportation. 
  • Personal Mileage Reimbursement: When public transportation is not available or does not meet your needs, you may be reimbursed personal mileage at 18 cents per mile up to $300 a month.
ModivCare Services

  • If you do not live in an area served by bus, paratransit services or you need door-to-door service, you may be eligible for ModivCare services. 
  • Contact ModivCare to schedule your ride at least 3 business days before your medical appointment.

UTA Reduced Farepay Card

    • UTA On Demand is in four zones located in Southern Salt Lake County, Salt Lake City Westside, Tooele County and South Davis County.
    • This is a corner-to-corner service, so you will be picked up and dropped off close to your destination. 
    • Wait times can range between 15-30 minutes when there is high demand for rides. If you have questions or need additional help booking a ride, contact the app customer service. If you are unable to book a trip on your second attempt, contact UTA customer service at 801-RIDE-UTA 801-743-3882 to get additional options.

  • To purchase a car seat, you must attend a car seat class and provide proof of qualifying income.
  • To qualify, family income must be at or below 195% of the current federal poverty level.
  • Also includes safety tips, videos, classes and safety checks.

Weber-Morgan Health Department Car Seat Program

  • Car seat safety class and an installation/check in your vehicle. Classes are free to residents of Weber and Morgan counties. Participants may receive a new car seat for free or purchase one at a reduced cost.
